Risk Warning & Waiver

By registering my child to participate in a PlayPerform program, clinic, or event, I acknowledge and accept the following:

• Football and related training activities carry inherent risks of injury, including from physical contact, equipment, uneven or slippery surfaces, and general physical exertion.

• My child participates at their own risk. While PlayPerform takes all reasonable precautions, to the extent permitted by law, PlayPerform and its staff, coaches, volunteers, contractors, and partners are not liable for injury, loss, or damage arising from participation - including in cases of negligence.

• I agree to follow all program rules and safety instructions. If I notice anything concerning about my child’s ability to participate safely, I will alert staff immediately.

Emergency Care

In the event of an accident or medical emergency, PlayPerform staff will attempt to contact the nominated emergency contact as soon as possible. If urgent care is required and a parent or guardian is not immediately reachable, I authorise PlayPerform staff to administer first aid and/or seek appropriate medical treatment on my child’s behalf.

Photography & Media Consent

By registering my child, I consent to PlayPerform taking photos or videos during sessions. These may be used for learning, internal review, or promotional purposes (e.g. website, flyers, or social media). My child’s name or identity will never be used without separate written consent. If I do not consent to photography, I will notify PlayPerform in writing prior to the program start date.

Instructor Substitution

If a scheduled coach is unavailable due to illness or unforeseen circumstances, PlayPerform may replace them with another qualified coach.

Refunds & Cancellations

• We do not offer refunds once payment is made, except in compassionate circumstances approved by management.

• If a session is cancelled by PlayPerform (e.g. due to extreme weather or coach illness), we will offer:

  • A rescheduled session, or

  • A credit toward a future service, or

  • A full refund if rescheduling is not possible

Program Changes Due to Unforeseen Events

If a session or program is cancelled due to circumstances beyond PlayPerform’s control (such as extreme weather, venue issues, or public health restrictions), PlayPerform will do its best to provide an alternative session, credit, or refund where appropriate.

General Agreement

By completing registration, I confirm I have read and understood these terms and conditions and agree to be bound by them.
